The intake interview is a meeting mostly for administrative purposes. On average, the intake interview is approximately 15-20 minutes long. The intake worker opens a medical chart for the new client and collects socio-demographic information as per Toronto Central Local Health Integration Network and the Ministry of Health and Long-Term Care requirements. The client is then provided with an appointment with a primary care provider.
For clients who do not attend an in-person meeting with the intake worker, an Intake Form and a Client Service Agreement is given to the client to fill out on their own. The intake worker, upon receipt of the completed intake form and signed service agreement, will open a chart for the client and contact them to book an appointment with a primary care provider.
For new clients to counselling services, the intake worker submits a counselling referral form to the counselling team, and the counselling team contacts the new client directly to schedule a counselling appointment.
