We offer a case management service for people who are homeless/under-housed, who are 30 years old and over who are clients of our centre. Service includes direct assistance such as referrals to and/or information about community resources, form(s) completion, advocacy, and accompaniments to appointments in the community.

Our service is a client-centered case management approach to ensure that the person who has experienced homelessness has a major say in identifying goals and service needs. The goal of case management is to empower the client, draw on their strengths and capabilities, and promote an improved quality of life by facilitating timely access to the necessary supports, thus reducing the risk of homelessness and/or enhancing housing stability. Our model of support is trauma-informed, which incorporates anti-oppression and harm reduction principles. We respect the diversity of our clients, and work to develop their capacity, focusing on their strengths.
